Ordering

The tumor analysis requires formalin-fixed, paraffin-embedded tumor tissue. Results of the DirectHit® Test Panel for Breast Cancer are available within six business days from the scheduled sample testing date.

If you are a treating physician and would like to order the DirectHit Test Panel for Breast Cancer, please follow the steps below.

  1. Complete DirectHit Test Panel for Breast Cancer Requisition Form

  2. It is the responsibility of the ordering physician to verify patient’s insurance information, coverage, contractual status, and out-of-network benefits of each policy. The Requisition Form must be accompanied by:

    • Copy of patient’s insurance card, front and back
    • Copy of patient’s driver’s license or photo ID
    • Letter of medical necessity from referring physician
    • Insurance authorization/verification sheet
    • Authorization letter (if required by the insurance plan), including the authorization number and authorized CPT codes, OR physician referral or prescription for testing
    • Signed Advanced Beneficiary Notice Form
    • Patient Financial Consent Form
    • Original signed documents

  3. Fax the completed Requisition Form to CCC Diagnostics, Inc. (410-633-4502). You will be contacted for account registration by CCC Diagnostics, Inc. Billing Department.

  4. Following account registration, a sample collection kit will be sent to your office via FedEx. Sample Kit Instructions Download

  5. Return samples via FedEx to:

  6. Patient results will be sent to the physician(s) within six business days from the scheduled sample testing date.
